What is a PHR?


A PHR is a Personal Health Record. It is a method of tracking and communicating your specific health information to healthcare professionals. The PHR can be used in a wide range of circumstances including Doctor visits, Dental check-ups, at the Pharmacy and of course in emergency situations. Is it true that President Bush said that every American must have a personal electronic medical record (EMR)?


Absolutely, as a matter of fact, President Bush said that if he could he would make it a law. By the year 2015 he mandates that every American must carry an EMR of some type with them. Also to bolster his Border Act, the person should have at least 3-5 forms of electronic un-alterable I.D. in it as well.

Who is responsible for keeping my information up to date and correct?

You are solely responsible for the information entered into your medical passport. Yourself and health care workers will utilize the information to make decisions about your medical care. It is critical that it be accurate and up to date.

Where is the data held?

Data is held on to the device.  In that way, no one has access to it unless you desire.

Will Medicare cover the cost of these?

At the present time Medicare is still reviewing the process and procedures and we will notify everyone if there is any changes.   You can use the cost as a health-related expense however.

Will doctors honor my Advanced Directives if they only have the MedMemory drive and not the paper copies?


Refer to the "State Regulations" for detailed information about each state. Obviously MedMemory cannot guarantee what physicians will do in various medical situations. However, while politicians write the laws, physicians practice medicine. The reality, in health care, is that rarely do physicians have access to all the legal copies of advanced directives and living wills. Generally in critical situations, physicians utilize every resource available – usually family, friends and primary care physicians – to make what they believe to be the best decisions for you.

Putting your choices and medical information in the MedMemory device and carrying it with you, gives physicians immediate access to your medical information and your choices about care. They can utilize this information first to care for you and if necessary to call your emergency contact person, Health Care Agent and if necessary verify your witnesses for the states legal purposes.

What if MedMemory does not meet all the requirements of my state regulations for advanced directives?

Refer to the "State Regulations" for detailed information about each state. Additionally we recommend you consult your physician and your attorney.

One of the guidelines in some states that can easily be addressed with MedMemory is to have your legal documents scanned digitally and attached to one of the entries.  Some state regulations are different. However, the state regulations are written to comply with the federal Patient Self-Determination Act. Although states differ in their approach to advanced directives, the intent is similar: competent patients can determine for themselves what health care they want and do not want. Physicians recognize and understand this right of patients.

Should I purchase a drive even if I have no medical problems?

Yes!  At the minimum, it provides a way for people to contact your loved ones during an emergency!

Can my doctor view my information after I leave his office?

It is up to you.  Doctors are only supposed to have information that you authorize them to have. 

What if I lose my CD or Flash Drive?

You should be backing up your entire USB regularly.

How can I get my medical records from my doctor, and will there be a fee?

Your physician is required by law to make available to you a copy of all of the information located in your file/chart. !!
